Step 1 - Intake Process

To file a complaint with the Office of Human Rights (OHR), an intake questionnaire must be completed and submitted to OHR to start the filing process.

Once you have completed the questionnaire you may submit the form online, or print to fax, mail or bring personally to OHR. Once OHR has received your intake questionnaire someone will contact you to schedule an appointment for an intake interview. Please select from the list below the intake questionnaire that pertains to your complaint. You may choose to fill out forms online or using the print version; both may be submitted online. (Please Note: Online forms work best with Internet Explorer "6" or earlier. Some page functions may not work properly with IE "7".)

If your complaint meets the jurisdictional requirement and other legal standards, the intake officer will generate a charge of discrimination and an affidavit. You will be required to declare that what you are alleging is true.

An intake counselor helps you draft the complaint into the proper format. The formal complaint form must be signed and notarized by you and copies will be mailed within 15 calendar days to each party.

Within 15 days after filing the complaint, the person or the organization against which you filed the complaint will be notified of your complaint by service letters and given a date of mediation. The “respondent” and you, the “complainant” will be asked to attend mediation within three to five weeks after your filing date.

Note: DC government employees alleging employment discrimination must first consult the EEO Counselor in their agency. The EEO Counselor has 21 calendar days in which to seek a solution on an informal basis. You may file formal charges with the OHR within 15 calendar days of your final interview with the EEO Counselor.
